Health Forms and Medical Information

  • Virginia School Entrance Health Form - must be turned in prior to the first day of school for:
    • Lower School - PK, JK, K, and new students 
    • Middle School - ALL sixth graders and new seventh- and eighth-grade students
    • Upper School - new students only
  • Asthma Action Plan - must be turned in each school year; for students prescribed an inhaler only
  • Allergy Action Plan - must be turned in each school year; for students prescribed Epi-pens only
  • Medication Request Form - must be turned in prior to medication being administered

Lower School:

2019-20 Milk Order Form; milk and lunch services begin September 3.

FA Café opens Tuesday, September 3:

  • Operates out of the kitchen on the first floor of the Middle School building Mondays through Thursdays and out of the kitchen on the second floor of the Upper School building on Fridays
  • Options include pizza, a salad bar, made-to-order sub sandwiches, drinks, and desserts (menus posted weekly)
  • Purchases are a la carte with cash (no checks for a la carte lunch orders, please), or Upper School students may purchase a $30 FA Café punch card from the Falcon's Nest

Supply Lists

From July 1 through July 25, families will have the option of purchasing grade level supply bundles online. The bundle includes all supplies available in the Falcon's Nest (personal items such as backpacks, lunch and snack bags, and other indicated items not included). These bundles are delivered directly to your student's classroom before the first day of school. Place your bundle order here.

Supply List Notes

There is no official supply list for Upper School students (with the exception of Algebra I and Geometry students; list coming soon). Students are expected to come to class prepared with the standard tools needed for organizing course materials and notes. Potential preferences may be notebooks, binders, loose-leaf paper, highlighters, pencils, pens, etc. Teachers may also give more specific recommendations for their classes in the first week of school.
